2022-2023 PATH REGISTRATION FORMS

All of the following items must be received before registration will be date and time stamped for processing:

*Placement tests are mandatory for Chemistry and all math classes for students that have not taken math here the year before. This will ensure students are in an appropriate class level and help the teacher know each student's ability. Tests are on our website on the front page or under Parent Resources and can be completed before you register if desired. Take the test that is the class you are interested in your child taking (instructions are included), scan the answers with any work shown and email to pathmilford@gmail.com or mail to PATH Milford P.O. Box 531 Milford, OH 45150. We will get them to the math teacher.
